Details

Heparin   is a natural anticoagulant, which exists in all mammals, so its name comes from its first isolation from liver tissue in 1916. Heparin is synthesized in mast cells and basophils and stored in the secretory granules of these cells. Because mast cells exist in many tissue types, heparin can come from this series of extrahepatic tissues. At present, heparin preparations often come from the mucosal intima of pig intestine.

Heparin can prevent blood from clotting because its unique pentasaccharide sequence contained in its structure is closely combined with antithrombin 3. Antithrombin 3 is a plasma protein that inhibits blood coagulation by binding with several activated coagulation factors, including Xia, Xa, IXA and IIA (thrombin). Heparin blocks fibrin formed by coagulation cascade, which is a necessary condition for blood clot formation. This anticoagulant effect of heparin can occur in vivo and in vitro.

Heparin sodium is a natural heparin salt used in medicine and laboratory. Heparin lithium is an in vitro anticoagulant specially used in the laboratory. It is prepared from heparin sodium by cation exchange chromatography. Heparin is the only anticoagulant used to prepare blood gas analysis samples. Heparin can interfere with the results in two ways. The first is the high concentration of heparin in the blood, and the second is heparin to dilute the blood if liquid rather than dry (freeze-dried) heparin is used.

Traditional blood gas analytes (pH, pCO2 and PO2) are less affected than electrolytes (especially ionic calcium), which is also measured by modern blood gas analyzers. Therefore, if only pH, pCO2 and PO2 are measured, the sample requirements are less stringent for heparin. For these analytes, it is still important that the heparin (sodium or lithium) concentration is less than 200 IU / ml blood and the blood dilution does not exceed 5%. The electrolytes included in the test track exclude the use of heparin sodium and the need for heparin lithium.

One of the common practical problems associated with blood gas analysis is insufficient anticoagulation and the formation of small blood clots, which will block the sample path of the blood gas analyzer and invalidate the results. Inadequate mixing of samples with heparin is often the problem. The lower the heparin concentration, the greater the risk of insufficient anticoagulation and related problems caused by poor mixing technology.
