Details

Since its discovery, heparin has been widely used in the prevention and treatment of various thromboembolic diseases because of its rapid onset, definite curative effect and reversal of anticoagulant effect. However, there are many kinds and similar names of heparin, such as heparin, low molecular weight heparin, enoxaparin and natroparin, which are easy to be confused with each other. What are the types of heparin and what are the differences between them Today, Desheng will show you the truth.

Different types of heparin

Heparin is mainly divided into unfractionated heparin (UFH), vascular additive heparin, low molecular weight heparin (LMWH), heparin derivatives (such as fondaparinux sodium) and heparin analogues (such as Dana heparin).

Unfractionated heparin is a sulfated mixture of glucosaminoglycans (GAGs). It is a mucopolysaccharide sulfate composed of D-glucosamine, l-eduuronic acid and D-glucuronic acid. It can be prepared from bovine lung or intestinal mucosa of cattle, sheep and pigs.

The anticoagulant heparin (heparin sodium or heparin lithium) for vacuum blood collection is an additive in the blood collection vessel. It is used in the anticoagulant tube and can prevent the rapid coagulation of blood in vitro after blood collection within a certain period of time. This kind of heparin is usually not injection grade, which is different from heparin drugs, but its effect price is relatively high.

Cosmetic raw material heparin can be added to cosmetics such as nutritional cream, eye cream, acne removing products and hair growth agents. It has many functions: increasing the vascular permeability of the skin; Improve local vascular circulation; Promote the supply of skin nutrition and the excretion of metabolic waste; It plays a good role in health care and maintenance of the skin.

Low molecular weight heparin is a short chain preparation separated from unfractionated heparin or degraded by unfractionated heparin. Due to the difference of molecular size, anticoagulant activity, preparation method and manufacturer, low molecular weight heparin commonly used in clinic includes enoxaparin, dalteparin and natroparin.

Differences between different heparins

Different molecular weight

The molecular weight of unfractionated heparin ranges from 3000 to 30000 Da, with an average molecular weight of 15000 Da, equivalent to 45 sugar units. The molecular weight of low molecular weight heparin is about 1 / 3 of that of unfractionated heparin, and the molecular weight of fondaparinux sodium is the smallest, only 1728da. The molecular weight is closely related to the anticoagulant activity and mechanism of action of drugs.

Different mechanisms of action

The anticoagulant effect of heparin drugs is mainly mediated by antithrombin Ⅲ (at - Ⅲ). Heparin molecule combines with at - Ⅲ lysine residue to form a reversible complex, which changes the configuration of at - Ⅲ, fully exposes the active site of arginine, and quickly combines with the serine active centers of factor IIA (thrombin) and IXA, Xa, Xia and XIIa to accelerate the inactivation of coagulation factor. When heparin passes through at - Ⅲ inactivating factor IXa / IIA, it must combine with at - Ⅲ and coagulation factor to form a ternary complex at the same time, while when inactivating factor Xa, it only needs to combine with at - Ⅲ. Once the heparin-at-iii-coagulation factor complex is formed, heparin is dissociated from the complex, combined with another molecule AT-III and reused

Different pharmacokinetic characteristics

Unfractionated heparin can be injected subcutaneously, intravenously or intravenously. Its bioavailability is low when injected subcutaneously, and its elimination in vivo is dose-dependent. In addition, the plasma protein binding rate of unfractionated heparin is as high as 80%, and most of it also binds to endothelial cells and macrophages, making its anticoagulant activity unpredictable. Compared with unfractionated heparin, low molecular weight heparin has obvious pharmacokinetic advantages. After subcutaneous injection, the bioavailability of low molecular weight heparin can reach 90%, and the plasma protein binding rate is low. Therefore, compared with heparin, the anticoagulant effect is easier to predict.

It should be noted that the heparin (heparin sodium and heparin lithium) currently produced and developed by Desheng is mainly the anticoagulant in the blood collection vessel, which can neither be injected nor used as a medicinal grade. Its potency is relatively high, generally between 150iu-180iu. Of course, it can also customize a higher potency. The higher the potency, the more expensive the price.
